Provide a signed report that contains this information:

• Bill of materials for the closure, fitment, liner, or safety seal product family. The BOM will list the resin family, resin density and MFI for each resin employed in the product family. The BOM does not have to contain any details describing the specific resin supplier or grade of resin. Materials listed on the BOM will be those listed in the APR Design® Guide for use with PET containers.
• Weight percent, or range of weight percent, that will be used in the closures, fitments, liners, or safety seals.
• Acknowledgement that technical data sheets for materials on the BOM will be kept on file for inspection should there ever be need to confirm compliance with MPG criteria.
